To coordinate and work with our partners and Local Area Coordination Teams to deliver a comprehensive Hospital Discharge Programme. Ensure that the Good Neighbour network in Derby City is able to support hospital discharge. Ensure effective administration systems are in place to deliver the service and development of the Derby Good Neighbour programme that will underpin effective delivery. Work with the VC team, Volunteers and our partners to ensure that monitoring, evaluation and recording systems are in place and followed.

• Hours: 35 (Full-time)
• Salary: £25,000
• Duration: subject to ongoing funding.
